Rename the file each month with your franchise code instead of "xxx", your name instead of "name", and the month and year (ex 0408) instead of "MMYY".
Add a line for each sale made. If you sell a customer both internet and telephone, use two separate lines.
Please put the last name first.
Also, indicate if you collected the $50 activation fee (which should be done any time you hand out a modem).
Finally, email your commission sheet to the dealer manager by the 5th of the following month. Example, April's commission sheet should be emailed to me by May 5th.
